Aspire's case managers assist individuals receiving services with locating, acquiring, directing, coordinating, and monitoring service provision to support both the services they need and the life they want.  Our case managers support participants in fulfilling their dreams for their best life including health and safety, community integration, paid employment services, and maintaining relationships. 

 

 

waiver services

A Kentucky Waiver Case Manager serves as a primary coordinator for individuals receiving Medicaid waiver services, helping them to access and manage supports that enable them to live in the community instead of an institution. Their responsibilities include identifying needs, developing a plan of care, arranging for services like those in the Supports for Community Living (SCL) or Michelle P (MPW) waivers, monitoring service delivery, and ensuring the participant's health and safety. They work with the participant, their family, and providers to create a person-centered plan

 

 

RISE Initiative


A Kentucky waiver case manager for the 1915(i) RISE Initiative helps participants by developing a Person-Centered Service Plan (PCSP), coordinating necessary services like housing and transportation, monitoring progress, and acting as a conflict-free point of contact within the complex service delivery system. They are responsible for linking individuals to various community supports, ensuring services meet the participant's specific needs, and advocating for them to reach their goals. 

 

 

Participant directed services

A Kentucky Participant-Directed Services (PDS) case manager helps individuals, who are enrolled in a Medicaid waiver program, to develop and manage a person-centered service plan and budget, find and hire service providers, monitor service delivery, and ensure the individual's health and safety. They serve as the participant's primary contact for PDS program, providing technical assistance and coordinating services to meet the participant's unique needs.
