Aspire's case managers assist individuals receiving services with locating, acquiring, directing, coordinating, and monitoring service provision to support both the services they need and the life they want. Our case managers support participants in fulfilling their dreams for their best life including health and safety, community integration, paid employment services, and maintaining relationships.
SUPPORTS FOR COMMUNITY LIVING
Individuals with the Supports for Community Living Medicaid Waiver utilize their case manager to coordinate assessments and evaluations, intake, referral and eligibility processes as well as the ongoing monitoring of service delivery. The case manager utilizes person centered planning processes in the identification and implementation of support strategies and ensure the encorporation of empowerment, community inclusion, health and safety assurances, and the use of formal, natural, and community supports.
MICHELLE P.
Individuals with the Michelle P. Medicaid Waiver utilize their case manager to coordinate assessments and evaluations, intake, referral and elibibilty processes as well as the ongoing monitoring of service delivery. The case manager coordinates the delivery of direct and indirect services and ensures that service delivery is in accordance with the individual's plan of care.
PARTICIPANT DIRECTED SERVICES
Participant Directed Supports (PDS) allows people eligible for Medicaid waiver services to choose their own providers for nonmedical waiver services. The case manager empowers individuals to define and direct their own services, develop their plan of care, and establish their budget allowance. The case manager also assists the individual with employee recruitment and hiring procedures.
