Hart Supported Living Grants

The Hart-Supported Living program is for Kentuckians with disabilities to request funds for supports so they can live in their homes and communities and avoid institutionalization.

 

National Core Indicators Project

NCI™ is a voluntary effort by public developmental disabilities agencies to measure and track their own performance.

The core indicators are standard measures used across states to assess the outcomes of services provided to individuals and families.  Indicators address key areas of concern including employment, rights, service planning, community inclusion, choice, and health and safety.

Services

The SCL waiver program was developed for Kentucky citizens as an alternative to institutionalization. The focus of SCL is to allow a person to remain in or return to the community. Persons with developmental or intellectual disabilities who meet the requirements for residence in an Intermediate Care Facility for persons with Intellectual Disabilities, and who meet other Medicaid requirements, are eligible for this program.

Supports for Community Living Waiver

Individuals with the Michelle P. Medicaid Waiver utilize their case manager to coordinate assessments and evaluations, intake, referral and eligibility processes as well as the ongoing monitoring of service delivery.  The case manager coordinates the delivery of direct and indirect services and ensures that service delivery is in accordance with the individual's plan of care.

Participant Directed Services

Participant Directed Supports (PDS)/Consumer Directed Supports (CDS) allow people eligible for Medicaid waiver services to choose their own providers for nonmedical waiver services. Provider choice gives members greater flexibility in the delivery of services received.
